Their second album The Height of Callousness (2000), produced by Garth Richardson, known for his work with Rage Against the Machine and Mudvayne, represented a significant leap forward. The record incorporated a stronger industrial metal influence and a more polished, relentless aggression, producing the singles Synthetic and New Disease. The band appeared on Ozzfest 2001 and toured alongside Slipknot, Linkin Park, Type O Negative, Disturbed, Hed PE, Orgy, and Mudvayne. The Height of Callousness is widely considered their definitive statement and one of the stronger records to emerge from the nu metal era.

Self-Destructive Pattern (2003) continued their Roadrunner run and produced the single Smothered, which earned a Grammy nomination for Best Metal Performance at the 46th Grammy Awards, losing to Metallica. Despite the nomination and the quality of the record, internal tensions led to Santos departing in 2004 to form Silent Civilian, and Spineshank went on hiatus. Santos returned and the band reformed in 2008, eventually releasing their fourth album Anger Denial Acceptance in 2012 through Century Media. In 2025, the band reunited again with Santos, Decker, and Garcia alongside new guitarists Tommy Decker Jr. and Jason Hager of ex-Chimaira to celebrate the 25th anniversary of The Height of Callousness.

The band's Grammy nomination, their Ozzfest appearances, and the enduring reputation of The Height of Callousness place them firmly among the most accomplished acts of that era, even if mainstream recognition never fully matched the quality of their best work.
